** Date: The Date shown in your client. Don't worry about timezone differences, just put your local time.

* If you are using the web interface
** Yahoo Direct Link: Insert the link to the page
** Submitted By: What is the email address of the person bringing up the issue

This helps the person who takes on the tracker to know where to find the issue in context, and to have a method of easily contacting the reporting/requesting party for clarifying details.
